  • [08B-38-4] Food stored on floor. Observed a box of cooked bacon and a box of tres leches stored on floor at walk in freezer. Manager stored properly.
  • [16-55-4] Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed DM off, as per manager waiting for new machine to arrived. Three compartment sink set up at time of inspection.
  • [12A-16-4] Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee touching dirty cups at bar area and proceeded to prepared coffee to customer without wash hands at bar area.
  • [14-11-5] Equipment in poor repair. Observed DM off, as per manager waiting for new machine to arrived. Three compartment sink set up at time of inspection.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed clean ice bucket stored by entrance kitchen door not inverted.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand sink by ware washing area. Employee provided.
  • [36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling by cook line soiled.
  • [42-01-4] Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored inside mop bucket by mop sink.
  • [53B-10-4] Records/documents for required employee training do not contain all of the required information. Observed missing date of completion. Manager complete form.
  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. By mop sink.
  • [01B-13-4] Stop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 5 packages of raw salmon at reach in cooler by storage thawed inside ROP. As per manager night before.
  • [10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed utensils stored inside standing water at 132°F by oatmeal. Manager discarded water.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed white cutting board at prep area across cook line heavily soiled.
  • [06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 5 packages of raw salmon at reach in cooler by storage thawed inside ROP. As per manager night before.
  • [25-27-4] Straws provided for customer self-service not individually wrapped or in an approved dispenser. Observed straws provided for customer at bar area not individually wrapped.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled by ice machine. Observed reach in cooler gaskets by kitchen area.
  • [02C-02-5] Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked pasta stored inside individual containers not dated at reach in cooler cook line. As per manager prepared two days prior. Manager dated.
