- [14-09-4] Cutting board has cut marks and is no longer cleanable.
- [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee backpack and sweater stored on rack above clean white plates and next to microwave. Items were moved to the office.
- [08B-38-4] Food stored on floor. Ice cooler storing ice that will be scooped for drinks observed stored on the floor in the front counter area. Discussed with manager on duty. Ice cooler remove off the floor.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets soiled with black substance.
- [27-14-4] Self-closing metering faucet does not provide water for at least 15 seconds. Not dispensing any water. Facilities employee came to change batteries on the faucet. Faucet now dispensing water.

A list of accredited food manager certification examination providers can be found at https://www2.myfloridalicense.com/hotels-restaurants/food-lodging/food-manager/ Observed expired training certificate for the kitchen employee Duane Lewis who was present during the inspection and was engaging in food preparation. Training was conducted on 2/28/2019 through ServSafe.
- [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. White cutting boards on reach in deli cooler stained yellow.
- [16-37-1] No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No quaternary test kit observed.
- [16-62-1] No measuring device available for measuring utensil surface temperature when using hot water as sanitizer in a dishmachine.
- [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Missing at hand handsink across from walk in cooler. Roll of paper towels place at the hand handsink.
- [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Could not be provided for employee present handling food. Discussed with manager and emailed a Food Employee Reporting Agreement form.
- [27-16-4] Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Handsink in prep area not dispensing any water. Hot water turned on.
