- [14-45-4] Cardboard used to line nonfood-contact shelves at clean pot rack.
- [13-03-4] Line cook with no hair restraint while engaging in food preparation. Cook put on hat.
- [08B-12-5] Stored food not covered. Observed assort cases cooked potatoes in walk-in cooler not covered or protected.
- [14-17-4] Walk-in cooler shelves with rust that has pitted the surface.
- [22-41-4] Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dishwasher machine at 000 ppm chlorine sanitizer. instructed operator they may use the machine to wash dishware and utensils, but must use three compartment sink to sanitize items once they come out of the dish machine and then allow items to properly air dry.

- [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed dice tomatoes at 48F and sliced cheese at 50F cold holding on make table at cooks line. Observed lid to cooler was left open, cook stay it had been open for at least 1 hour. Cook closed lid on cooler.
- [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink in kitchen. Oper replaced pap towels.
