  • [14-11-5] Equipment in poor repair. Gaskets on both doors of flip top reach in cooler on cook line torn.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing at the end of the splitter for mop sink faucet where black hose is attached.
  • [35A-02-7] Live, small flying insects found. Observed 1 live flying insect near bag- in- box soda rack. Also, observed 1 live flying insect near mop sink.
  • [41-10-4] Toxic substance/chemical improperly stored. On bottom shelf under front counter, spray bottles of disinfectant and sanitizer and cans of stainless steel cleaner stored next to packs of napkins. Employee moved all chemicals to proper storage area.

Sliced provolone is stacked higher than fill line of pan. Employee removed some cheese and put in bottom of reach in cooler to bring temperature down.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. In walk-in cooler three boxes of raw beef for sandwiches on shelf above ready to eat bacon bits and sliced turkey. Employee put boxes of raw beef on bottom shelf and moved all ready to eat food to top two shelves.
  • [25-05-4] Single-service articles improperly stored. In back dry storage, box of paper towels stored on floor.
