At make line, times missing on cut lettuce, cut tomato, sliced cheese. Employee stated items placed in service 20 minutes prior. Operator placed times on all items.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. At flattop area, side of fryer has grease build up. At drive thru, tray of soda dispenser has mold-like substance at drain area. At main fry station, top of hood over hot holding bin soiled with food debris/residue. At main fry station, gasket on reach-in freezer has food debris.
