- [14-11-5] Equipment in poor repair. Air conditioning in kitchen not functioning. Portable air conditioners are in use while awaiting repairs.
- [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice bin has mold-like substance in corners, along sliding door grooves, and in interior chute.
