Operator removed cushion from resting on reach in freezer and discussed repositioning the small partitioning that was covering a table.

Multiple bins stored in back storage room, containing white powdery substance that have been removed from their original packaging not labeled. Operator started to label the flour and sugar in back storage room.
- [16-48-4] Old food stuck to clean dishware/utensils. Observed old food stored on clean knives that were stored on clean knife rack. Operator removed unclean knives to have washed.
- [25-06-4] Single-service articles not stored inverted or protected from contamination. Observed single service to go trays on dry food storage shelves not inverted/ protected.
- [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee personal belonging stored next to single service souflee cups near booth on side dining area. Operator removed employee back pack to properly store.
- [31A-11-4] Handwash sink used for purposes other than handwashing. Observed hand wash sink on front service line used to store trash. Operator removed trash from hand wash sink.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed exterior of equipment on cook line and in prep area soiled. Also, observed gasket on reach in freezer in prep area and back room soiled. Also, exterior of large trash can next to triple sink heavily soiled.
- [36-64-5] Objectionable odors in bathroom or other areas of the establishment. Observed objectionable odors in back room and both restrooms.
- [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw,chicken stored over ready to eat chicken nuggets in reach in freezer across from microwave. Also, observed raw beef stored over cooked pork in reach-in cooler in prep area. Operator rearranged products in both reach-in cooler and reach in freezer to properly store. Also, raw chicken stored above raw beef in reach in deep freezer in back storage room.
