- [12A-27-4] Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed an employee crack raw shell eggs than handle clean utensils without changing gloves or washing hands. Operator washed their hands and changed their gloves.
- [14-09-4] Cutting board has cut marks and is no longer cleanable. Observed their cutting boards partially melted and bent.
- [31A-11-4] Handwash sink used for purposes other than handwashing. Observed an employee dump a sanitizer bucket into a hand wash sink. Reviewed with the employee the proper use of the sink.

- [03F-02-5] Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed no time mark for their 4hr shell eggs on the cook line. Operator stated they had been placed out approximately 1-1/2hrs ago. Operator labeled their chart accordingly.
- [10-06-5] In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed their scoop handle buried in their potatoes. Operator removed it.
- [22-41-4] Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the dishwasher machine at 0ppm chlorine. Operator worked with the pipes and got it reading 100ppm chlorine.
