  • [14-45-4] Cardboard used to line nonfood-contact shelves. Behind table at front counter and at drive thru area, plastic sign board used as back splash. Board has hollow core resulting in gaps with food debris.
  • [21-08-4] Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer buckets at front counter and cook line, Sanitizer Bucket (Quaternary 0ppm). Manager remade both buckets and rechecked at 150ppm.
  • [12B-12-5] Employee drinking from an unenclosed beverage container in a food preparation/clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container. Employee open drink cup of fruit punch on table on cook line. Manager removed drink and discarded.
