[16-62-1] No measuring device available for measuring utensil surface temperature when using hot water as sanitizer in a dishmachine. Observed operator unable to provide a measuring device to test sanitizer in high temp dish washing machine.
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed multiple portable cutting boards in prep area soiled/ stained. Also, Vegetable wedger stored on clean storage shelf in prep area, and blade of can opener on cook line soiled.

Observed cooler have large mass of ice build up covering fan area. Operator stated items have been in cooler overnight. Observed house made creme brulee, house made cheesecake, house made key lime pie (45F-49F - Cold Holding) in reach-in cooler across from steam table on cook line. Observed cooler have large mass of ice build up covering fan area. Operator stated items have been in cooler overnight. Unsure of when cooler stopped being able to maintain temperature of 41F and below. Stop Sale. Also, milk, half and half (52F - Cold Holding) in glass door reach-in cooler on front service line. Employee stated cooler door has been open and closed quite a bit today. Operator stated he is looking into getting a new cooler. Also, house made ranch dressing (52F - Cold Holding) in reach-in cooler at expo line. Operator stated items went in cooler this morning. Operator removed dressing and placed in nearby cooler to chill
[29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed two threaded hose bibs in front of ware washing area without vacuum breakers/ backflow preventers.
[14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up around walk in freezer door, floor and on some of the boxes in freezer.

[31B-05-4] Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Paper towels unable to dispense at dispenser in meat cutting room. Operator readjusted paper towels to properly dispense.
