Operator stated they were in the cooling process and were prepared three hours ago. Advised operator to place rolls in walk in cooler, operated placed in walk in cooler.
[12A-03-4] Employee washed hands in a sink other than an approved handwash sink. Observed employee washing hands at three compartment sink.
[02C-08-5] Ready-to-eat, time/temperature control for safety food opened/prepared, frozen and then thawed and held at refrigeration temperature not properly date marked. Various cooked meats in walk in cooler being thawed with no date mark of when food was prepared and frozen.
[08B-38-4] Food stored on floor. A total of 3 metal trays of raw chicken are being stored on the floor of the walk in cooler. Operator removed foods from the floor and stored properly.

[35A-05-4] Roach activity present as evidenced by live roaches found. One live roach observed on prep table of kitchen underneath rice warmer. Operator killed roach and cleaned and sanitized area.
