  • [14-11-5] Equipment in poor repair. Metal seal of door gasket to walk in cooler torn. In walk in freezer ice build up forming under compressor unit. Steam table water gauge with disintegrating cork attachment. Per operator has requested replacement.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed residue build up on front counter ice tea dispenser nozzle.
  • [24-07-4] Cleaned and sanitized equipment or utensils not properly stored. In exterior storage shed multiple metal trays stored on concrete floor.
  • [25-05-4] Single-service articles improperly stored. Box of single service fry containers stored on floor in front of dry storage hand washing sink.
  • [33-16-4] Open dumpster lid. Dumpster sliding lid door left open.
  • [36-17-5] Floor tiles missing and/or in disrepair. Floor tiles of mop sink cracked and or missing from base of mop sink.

  • [12A-13-4] Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee on cook line remove gloves, place in trash can, handle broom to sweep floor, adjust hat, then without washing hands, begin food prep handling fryer baskets. Discussed with operator observations.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Warming unit over fry baskets with food debris and grease buildup. Drive through soda dispenser with mold like substance on underside plate behind nozzles. Produce reach in cooler bottom door gasket has mold like substance build up.
  • [35A-03-4] Dead roaches on premises. Observed dead roach like insect on floor of office. Operator swept and discarded dead roach like insect.
