Employee at cookline stated the mushroom gravy was thawed inside the walk-in cooler and placed into the reach-in cooler approximately 10 minutes prior. Inspector had the manager move the mushroom sauce to the freezer to rapidly cool to 41F.

Inspector provided manager a new copy of the employee health reporting agreement during this inspection.

Hole in wall below handwash sink at cookline. 4. Gaps in wall at corner near three-compartment sink. 5. Vents on ceiling throughout kitchen rusted. 6. Interior of cabinet below fountain machine rusted. 7. Door release button broken and missing on interior of walk-in freezer door.
