- [10-03-4] In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away. In dipperwell on server line, top of scoops not covered in water. Employee filled well with more water.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets soiled with food debris on cook line. Also, tables under cooking equipment soiled with food debris build up.
