Eggs were placed in cooler no more than 1 hour prior but lid to cooler has been open for rush. Manager put ice on eggs and shut lid to cooler.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets throughout establishment soiled/torn. Also, around nozzles to slushee machine soiled with black residue. Also, top of cooler under grills soiled with black debris/food debris.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Two ice buckets on top of ice machine not inverted. Manager flipped ice buckets over.
  • [38-05-4] Ten (10) foot-candles of light at least 30 inches from floor not provided in dry food storage area. In storage room with bag- in- box soda rack light is not working.
  • [25-06-4] Single-service articles not stored inverted or protected from contamination. To go cup lids at front counter and drive thru not inverted. Employees flipped stacks of lids over.
  • [05-09-4] No conspicuously located ambient air temperature thermometer in holding unit. No ambient thermometer in flip top reach in cooler on cook line.
  • [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Ice build up on door of inside walk-in freezer.
  • [36-51-4] Building components, attachments or fixtures in poor repair. Multiple hood filters broken/in disrepair. Manager stated new hood filters are ordered and on the way already.
  • [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Interior of flip top reach in cooler on cook line and reach in cooler under French fry warmer soiled with food debris.
  • [33-20-4] Grease/trash on the ground and/or pad around grease receptacle. Grease on ground in dumpster enclosure. Also, trash on ground in dumpster enclosure.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In bottom of flip top reach in cooler on cook line, bag of shredded cheese (49F - Cold Holding). Bag of cheese had been out for restocking purposes then placed back in reach in cooler.
