  • [11-27-4] Discussed printing Clean-Up of Vomiting and Diarrheal Events (DBPR HR 5030-104) form. Establishment has chlorine associated with written procedures.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets soiled with mold like debris. Exterior of fryers soiled with grease build up.
  • [24-08-4] Clear plastic cups stored on shelf next to soda dispenser.
  • [36-27-5] Wall soiled with accumulated grease, food debris, and/or dust. With black mold like substance above three compartment sink.
  • [22-02-4] Can opener blade soiled with dried food debris. Employee stated it isnt being used. White cutting stained yellow on reach in deli cooler at end of cook line.
  • [51-16-7] Establishment is operating as Frog Pond for morning service from 7:30am-1:30pm and reopens as MadFish for evening service 5pm-10pm. Frog Pond and Madfish are two completely separate concepts with different menus, individual staff teams, and distinct operating hours, with Frog Pond serving breakfast and Madfish serving dinner. Outdoor storage cold holding cooler trailer and a dry storage trailer have been observed in the parking lot of the establishment. Violation will be time extend for 60 days from initial inspection on 7/23/2026. Callback date will be on 9/21/2026.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. White cutting board stored at in reach in freezer on cook line.
  • [16-37-1] Chemical test kit has been ordered and will be arriving tomorrow.
