  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed missing vacuum breaker non-chemical side of hose splitter at mop sink.
  • [16-46-4] Old labels stuck to food containers after cleaning. Observed old labels stuck to clean containers stored across from three compartment sink.
