- [36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Multiple ceiling tiles in the dining room show signs of water damage, some black-mold like substance observed on some tiles. Manager states the ceiling tiles are due to be replaced/repaired today.
- [35A-03-4] Dead flies on premises. Approximately 20-30 dead flies along windows in dining room along side entry door.
- [36-22-4] Floor area(s) covered with standing water. Standing water observed in two areas of the dining room, in front of the lobby drink machine and at first round table at lobby drink machine. Also, standing water observed behind toilet in large stall of womens restroom.
- [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up observed in french fry reach-in freezer and the walk-in freezer at the fans.

- [36-50-4] Unclean building components, attachments or fixtures. Fan guards in walk-in cooler have dust.
- [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Raw bacon in reach-in cooler on make line at 46F. Employee states they cooked bacon in the last few minutes. Bacon may have been left out of refrigeration for a few minutes. Bacon moved to walk-in freezer and approximately 10 minutes later, bacon is at 41F.
- [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink by the washer, vacuum breaker missing from non-chemical side of attached splitter with attached green hose.
