Flying insect is landing on multiple kitchen prep tables and on the reach in cooler lids for pizza mix station, salt and sugar containers next to three compartment sink. Employee removed flying insect from inside MFDV at time of inspections.
  • [08B-49-4] Employee personal food not properly identified and segregated from food to be served to the public. Several bags of ice for employees personal use not segregated or identified in the kitchen reaching freezer. Employee removed ice at time of inspection.

Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed multiple large wood structures built in front of and on the side of the MFDV, multiple raised plant beds on ground surrounded the mfdv, a restroom built with wood enclosure next to the MFDV. A wooden wall with flowers and soil in front of towing hitch so its unable to be towed. 2 large propane tanks and grease trap store on ground at exterior of food truck.
