  • [36-34-5] Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents dusty at cook line.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed ice buckets stored not inverted at ice machine. Operator inverted buckets.
  • [14-09-4] Cutting board has cut marks and is no longer cleanable. Along cook line.
  • [36-22-4] Floor area(s) covered with standing water. Observed standing water on floor on cook line, dish and bar areas.
  • [10-03-4] In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away. Observed dipperwell not completely on by rice cookers, operator turned on water.
  • [02D-01-5] Working containers of food removed from original container not identified by common name. Observed containers of sugar not labeled under prep table in back area.
  • [22-49-4] Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dishwasher (Temperature 154F). Operator reset dish machine for final temperature Dishwasher (Temperature 160F).
