  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice bin at ice machine in back kitchen area soiled.
  • [35A-02-8] Live, small flying insects found. Observed one flying insect in back kitchen area.
  • [48-04-4] Propane tank (larger than 2.7 lb. water capacity/1 lb. gas capacity) located inside of the building. For reporting purposes only. Observed propane tank in back kitchen area.
  • [50-17-3] Operating with an expired Division of Hotels and Restaurants license. Establishment observed preparing, serving, or selling food to customers with an expired license. Establishments license expired 6-1-26.
  • [10-03-4] In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away. Observed dipper well for ice cream scoops insufficient velocity/turned off. Operator turned it back on.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Observed souflee cups used to dispense nuts on counter next to glass door reach-in cooler. Operator removed souflee cups.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed white ice making plate/ deflector plate in ice machine in kitchen area soiled.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink at front service area and also no paper towels at hand wash sink next to ice machine. Operator brought over some napkins to hand wash sink at front line area.
  • [41-17-4] Spray bottle containing toxic substance not labeled. Observed unlabeled Spray bottle containing toxic substance stored at triple sink. Discussed with operator to label the bottle of bleach.
  • [08B-38-4] Food stored on floor. Observed bag of sugar and whittle waters in plastic bottles stored on floor next to dry food storage shelf. Operator removed both foods to properly store.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sour cream, cut tomatoes, cut cabbage (44F-46F - Cold Holding) in glass door reach-in cooler at front line area. Operator stated she had some of the foods out during lunch. Operator stated she will leave the door closed for the cooler to chill the foods. Retemped at end of inspection Cut cabbage and cut tomatoes 42F-43F, sour cream 45F
  • [53B-01-5] No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed person in charge unable to provide proof of employee food safety training. Person in charge stated she and one other employee have been here more than 60 days.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator unable to provide proof that employees are informed of reporting symptoms related to food borne illnesses to person in charge. Distributed health form to operator during this inspection and employee on duty read and signed form.
