  • [35B-01-4] Exterior door has a gap at the threshold that opens to the outside.
  • [24-13-4] Clean utensils or equipment stored in a bathroom, locker room or mechanical room. Observed clean bowl, plate and to go container in employees bathroom. Operator removed it.
  • [36-27-5] Wall soiled with accumulated grease, food debris, and/or dust.
  • [03B-01-6] Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed meatballs (120F - Hot Holding); white rice (110F - Hot Holding); sancocho (109F - Hot Holding); yuca (120F - Hot Holding); cooked beef (113F - Hot Holding), for less than 4 hours as per operator. Operator reaheated at 165F .
  • [36-24-5] Hole in or other damage to wall. Bathroom wall and underneath of three compartment sink.
  • [02C-02-5] Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked beef, dessert, sancocho, was prepared from more than 24 hours as per operator. Operator covered it during inspection.
  • [31B-04-4] No handwashing sign provided at a hand sink used by food employees.
