- [12A-29-4] Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee touched cell phone while wearing gloves and then handled clean utensils and container of food without washing hands and changing gloves. Discussed with employee and he removed gloves and washed hands.

Observed gaskets on reach in deep freezers in back room, gasket on reach-in cooler in prep area near kitchen entry door soiled. Also, shelving in prep area down middle of kitchen have build up of dust and debris. Also, interior of hand wash sink on front service line soiled. Operator cleaned sink basin during this inspection.
- [31A-11-4] Handwash sink used for purposes other than handwashing. Observed hand wash sink on cook line used to store supplies, utensils. Employee removed all items from hand wash sink.
- [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed multiple bowls and pots and pans in storage area on shelving down middle of kitchen and across in clean storage area not inverted/ protected.
- [08A-02-6] Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Observed raw pork stored over ready to eat spring rolls in reach in freezer in back storage area-not all products commercially packaged. Operator rearranged items to properly store.
- [33-02-4] Cardboard box used as garbage container. Observed multiple boxes used as garbage cans in kitchen and on cook line.
- [01B-01-4] Dented/rusted cans present. See stop sale. Observed can of green beans on dry food storage shelf dented and crimped along rim and side of can. See stop sale.
