[12A-29-4] Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee touched cell phone while wearing gloves and then handled clean utensils and container of food without washing hands and changing gloves. Discussed with employee and he removed gloves and washed hands.

[36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Observed multiple ceiling tiles in back storage room in disrepair/ water damaged. Also, multiple floor tiles in kitchen area, warewashing area and cook line cracked/ in disrepair.

[09-01-4] Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee handling cooked shrimp, cut lettuce, and carrots with bare hands, ingredients not being heated to a minimum of 145F. Establishment doesnt have an approved alternative operating procedure. Discussed with employee, employee washed hands and placed on gloves.
