Palettes made in house and sold for customer self service on grocery side missing minimum labeling.
  • [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Hose bibbs at mop sink missing vacuum breaker. One side missing vacuum readers from both sides of splitter.

In restaurant side raw ground beef stored over cooked beans. In reach in freezer raw chicken no longer commercially packaged stored over uncovered tray of chocolate covered bananas. Person in charge began rearranging.
  • [01A-05-4] Food/ice received from unapproved source/no invoice provided to verify source. See Stop Sale. Observed desserts on site from Jackies Kitchen per sticker. Hand written invoice provided. Unable to provide proof establishment is licensed. Not observed on FDACS search. Person in charge unable to get a hold of supplier during inspection.
  • [36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tile in back kitchen with damage. Also, floor with debris accumulation around water heater, triple sink area.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Cutting boards at make line, in clean storage area with debris accumulation.
  • [05-08-4] No probe thermometer provided to measure temperature of food products. Observed thermometer provided not functional.
