[03F-02-5] Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control. Sushi rice held on Time as a Public Health Control not time marked. Per operator, rice prepared approximately 1 hour ago. Rice was time marked.
[21-09-4] Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean. Soiled wet wiping cloths stored on flip top. Advised employee to use clean cloths and place in sanitizer bucket.
[14-09-4] Cutting board has cut marks and is no longer cleanable. Cutting board stored on rack in kitchen with deep grooves.
[25-06-4] Single-service articles not stored inverted or protected from contamination. Stack of single service containers stored upright on box next to 3 door freezer in kitchen. Containers were inverted.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In bottom of flip top # 1, in kitchen, Cooked Pork Wontons (53F - Cold Holding). In flip top # 2 in kitchen, Fried Rice (62F - Cold Holding). Per operator, foods not prepared or portioned today. Foods held in walk in cooler and moved to flip top coolers and held for approximately 2 hours. Foods were moved back to walk in cooler to quick chill.

[29-09-4] Faucet/handle missing or damaged at plumbing fixture. Hot water handle on hand sink in dish area stripped.
[08B-38-4] Food stored on floor. 1) Buckets of peeled onions on floor in walk in cooler. 2) Container of fryer oil on floor in kitchen prep area.
[08B-17-4] Unwashed fruits/vegetables stored with ready-to-eat food. Avocados with stickers still on them stored in sushi reach in cooler with prepared cut vegetables. Employee washed and removed stickers.

[41-17-4] Spray bottle containing toxic substance not labeled. Unknown substance in pump spray bottle on floor by dish machine. Employee labeled as degreaser.
[27-16-4] Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. At hand sink in dish area due to damaged faucet.
