[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the ice chute on the soda machine is soiled. ALSO, the can opener blade has dried food debris at the top of the blade.
[22-43-4] Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed the 3 compartment sink tested at 0ppm for quaternary ammonia. The water was tested multiple times. The water was then dumped and refreshed to 200 ppm.
[16-37-1] No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed : lack of available test strips at the 3 compartment sink. The test strips are already partially green(originally orange) and may have been wet before.
