Observed imitation crab (73F - Cold Holding) inside 3 compartment sink , as per operator less than 2 hours, operator moved to walk in cooler. Also observed raw tuna (44F - Cold Holding); raw salmon (44F - Cold Holding); at sushi station, as per operator door was opened less than 1 hour.
  • [10-12-5] In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored inside soiled with mold like substance holder next to ice machine, operator moved to dishwashing area.
  • [23-24-4] Buildup of food debris/soil residue on equipment door handles. Observed soiled gaskets on salad reach in cooler next to expo area.
  • [24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed ice bucket stored not inverted next to ice dispenser at dine in.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal bag on preparation table at expo area. Operator removed.
  • [42-01-4] Wet mop not stored in a manner to allow the mop to dry.
  • [35B-01-4] Exterior door has a gap at the threshold that opens to the outside. Observed at back door.
  • [41-17-4] Spray bottle containing toxic substance not labeled. Observed degradation spray bottle not identified next to mop sink.

  • [06-06-5] Frozen time/temperature control for safety food being slacked at room temperature is no longer frozen solid. Observed imitation crab (73F - Cold Holding) inside 3 compartment sink , as per operator less than 2 hours, operator moved to walk in cooler.
  • [12A-17-4] Employee rubbed hands together for less than 10-15 seconds while washing hands. Coached employee on proper washing practices.
  • [36-01-4] Floor not cleaned when the least amount of food is exposed. Observed under rack at dry storage area.
  • [12A-07-5] Employee failed to wash hands before putting on gloves to initiate a task working with food. Coached employee on proper procedures.
  • [08B-12-5] Stored food not covered. Observed white rice bin not covered at rice station.
  • [51-18-6] No copy of latest inspection report available.
  • [22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Next to dry storage area.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. Observed next to dishwasher machine. Also at sushi station.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Observed on panko container at rice station.
  • [21-10-4] Soiled dry wiping cloth in use. Observed multiple soiled towels at sushi bar station.
