Observed 4 dead roaches in the drain under prep sink. Observed two dead roaches in the grate with plug in back of small reach in freezer in kitchen. Observed one dead roach on the framing of walk in freezer door. Observed three dead roaches on the floor in front of walk in freezer. Observed approximately 5 dead roaches on the floor in liquid debris by kitchen door leading to front counter. Observed approximately 5 dead roaches in a glue trap under front counter hand wash sink. Observed approximately 5 dead roaches in brown liquid debris on floor at front counter area. Person in charge began cleaning.

[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cooked onions, cheese, sausage in small makeline across from pizza oven at 47-49F. Person in charge states had unit unplugged for approximately 1 hour to clean. Ambient temperature of unit 49F during inspection. Person in charge moved time/temperature control for safety items to other unit holding temperature.
[36-36-4] Ceiling tile missing. Multiple ceiling tiles missing over pizza oven. Also, staining/ grease accumulation observed on ceiling and wall in kitchen by pizza oven. Also, damage to cove molding in corner by green shelving to left of triple sink. Also, no smooth and easily cleanable ceiling at secondary dry storage to left of main cash register at entrance to building. Also, hole in the wall at triple sink where piping is running through wall. Also, several areas of the floor not smooth and easily cleanable.
