  • [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Interior of two door reach in cooler at end of cookline has food debris and spilled liquid build up at base of cooler. Interior of bulk ice bin has rust build up on deflector plate corners.
  • [12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink stored on shelf over bagged bread.
  • [10-06-5] In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Hashbrowns scoop handle buried in product in cookline reach in cooler. Operator relocated.
  • [14-33-4] Reach-in cooler shelves with rust that has pitted the surface. Glass door reach in cooler for the waitstation has rust pitting the surface of the shelves.
  • [21-17-4] Clean linens stored on floor. Bags of clean linen stored on the floor by bulk ice bin. Operator began relocating.
  • [29-03-4] Water draining onto floor surface. Mid cook line soda ice bin has bucket placed under ice well to catch run off water, bucket to catch water over full and leaking to floor.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Cookline hood filters with grease build up forming. Food debris at end of cookline between grill area and reach in cooler.
  • [53A-05-6] No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.   • [25-05-4] Single-service articles improperly stored. Box of single service carry out container lids stored on the floor in front of the hand wash sink in back prep area.
  • [16-46-4] Old labels stuck to food containers after cleaning. Old labels on clean plastic containers over three compartment sink.
  • [41-15-5] Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Sanitizer solution at end of cookline at over 200ppm chlorine, strip bleached back to white. Employee reset to 100ppm.
  • [08A-11-5] Raw animal food stored over canned/bottled drinks. In two door reach in cooler at end of cookline line, raw shell eggs over cans of tomato juice and containers of milk.
