  • [35A-02-8] Live, small flying insects found. 1 live small flying insect in kitchen by warewashing area.
  • [36-36-4] Ceiling tile missing. Light tile by walk-in cooler is moved leaving a gap. Per operator they are working on repairing.
  • [36-32-5] Ceiling/ceiling tile shows damage or is in disrepair. Vents in kitchen rusted and paint chipping, dust around tiles in vent by hostess station.
  • [25-22-4] Single-service articles stored next to handwash/food preparation sink exposed to splash. Small plastic cups for butter at server station below paper towel dispenser.
  • [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee sweater stored on top of bread buns at bottom of prep table on cook line. Operator removed.
  • [12B-13-4] Opened employee beverage container in a cold holding unit with food to be served to customers. Dr Pepper can inside reach-in freezer in kitchen, and employee cup in reach-in cooler at bar. Operator removed.
