  • [10-02-4] In-use utensil not stored on a clean portion of food preparation or cooking equipment. Cardboard used to store in-use tongs at cookline. The manager discarded the cardboard and replaced the tongs during this inspection.
  • [14-71-4] Duct tape used to repair nonfood-contact surface. Duct tape on handles of several food bins throughout kitchen.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Exterior of rice bin at cookline heavily soiled with grease and/or food debris. 2. Handles, front, and gaskets of reach-in cooler at cookline heavily soiled with grease.
  • [29-20-5] Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Standing water in mop sink. Inspector ran water at each other sink and all other drains appear to be draining properly.
  • [33-11-4] Missing drain plug at dumpster. Drain plug missing from dumpster behind restaurant.
  • [36-37-5] Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Absorbent and porous acoustic ceiling tiles in-use throughout kitchen.

  • [08A-04-5] Raw animal food stored over or with unwashed produce. Pan of raw chicken stored above box of broccoli inside reach-in cooler in back of kitchen. The manager moved the raw chicken to a lower shelf during this inspection.
  • [14-45-4] Cardboard used to line nonfood-contact surface. Cardboard used to store in-use tongs at cookline. The manager discarded the cardboard and replaced the tongs during this inspection.
  • [14-38-4] Food storage container/container lid cracked or broken. Lid of rice bin at cookline cracked.
  • [36-50-4] Unclean building components, attachments or fixtures. Wire shelving on walls throughout kitchen soiled with grease.
  • [35A-02-7] Live, small flying insects found. 1. Two flies flying in kitchen at cookline. 2. One fly flying in dry storage room. 3. One fly crawling on pipe below handwash sink near three-compartment sink. 4. One fly crawling on floor in center of kitchen.
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Empty box sitting on top of handwash sink at cookline. An employee removed the box from the area during this inspection.
  • [41-07-4] Container of medicine improperly stored. Pill bottle stored on shelf above single-use trays and above refrigerated make-table containing food at cookline. The manager removed the pill bottle from the area during this inspection.
