[14-11-5] Equipment in poor repair. Observed torn gaskets on two reach in cooler in the kitchen, observed walk in freezer door does not fully closed.
[35A-02-7] Live, small flying insects found. Observed approximately 20 small flies in dry storage over two bags of onions and observed approximately 10 flies on the prep table and dish area.
[40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed an employees phone on a cutting board in the prep area.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on the prep table Shredded mix cheese-71F, Guacamole-69F, Diced tomatoes-63F, Cut lettuce-61F, sliced onions-62F and sour cream-72F, employee stated has been there since 10:30 am. [12A-09-4] Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook touched his apron while cooking and not change his gloves, operator spoke to the cook and educated on proper hand washing and changing gloves.
