- [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. At wait station drink station, cutting board has black stains. Operator voluntarily discarded.

Operator stated it is used as a prep sink and sink at kitchen entrance is used for handwashing. Handwash sink in prep area is on plans as a handwash sink.
