- [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. At mop sink, 4 way splitter had missing vacuum breaker on one end. Operator placed vacuum breaker on splitter end. At dumpster enclosure, hose bib missing back flow preventer.

Observed, multiple gaskets on reach in cooler and reach in freezer soiled with food debris and black/green mold like substance throughout kitchen. Operator began cleaning gaskets Observed on the kitchen bulk ice bin and drive thru bulk ice machine, black/green mold like substance on exterior lids of both bulk ice bins. Operator cleaned exterior lids. At pass thru window fry warmer, the heating element area soiled with grease build up.
- [22-16-4] Reach-in cooler interior/shelves have accumulation of soil residues. Observed in raw chicken reach in cooler next to chicken breading station, shelves soiled with food debris/residue.
- [08B-17-4] Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, cases of unwashed strawberries over ready to eat food. Operator reorganized and put strawberries on bottom shelf.
