Several new employees working in kitchen and handling food without proof of food handler training or proof they have been informed of their health reporting responsibilities. Inspector provided manager a new copy of the employee health reporting agreement during this inspection.
- [40-06-5] Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employees personal bag stored directly on top of box containing restaurant food at far end of kitchen. Manager removed the item from the area during this inspection.
- [41-10-4] Toxic substance/chemical improperly stored. Container of "Clorox" powder stored on shelf directly beside clean dishes and open box containing single-use foil liners. Manager moved all dishes and single-use items away from chemicals during this inspection.
- [42-03-5] Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. 1. Screwdriver stored on wire shelving, directly above containers of seasoning in kitchen. Manager removed the screwdriver from the area during this inspection. 2. Roll of duct tape stored on prep table directly beside milkshake mixer in kitchen. Manager removed the duct tape from the table during this inspection.
- [16-46-4] Old labels stuck to food containers after cleaning. Remnants of old labels stuck to sides of 4 pans on clean dish rack.
- [29-42-4] Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Backflow prevention device missing at green hose connection after splitter added to mop sink.
- [13-07-4] Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Several employees handling food in kitchen while wearing bracelets and/or watches on wrists.

Two open jugs of milk missing date marks inside reach-in cooler near kitchen entrance. An employee stated the jugs of milk were both opened three days prior and added the proper date marks during this inspection.
