- [10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed rice paddle in standing room temperature water (81°F). Operator cleaned cleaned, sanitized, and stored in water maintained at 135F+.
- [14-11-5] Equipment in poor repair. Observed flip top cooler in poor condition. Overnight temperature 43F on all foods. Operator to utilize ice bags on TCS foods today until repair technician can correct to 41F or lower.
- [08B-47-4] Food not stored at least 6 inches off of the floor. Operator placed containers of chicken directly on the floor.
- [02C-04-5] Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed frozen on site prepared food marked with preparation date, but no additional information of when moved from freezer to refrigeration.
