  • [01B-02-5] Stop Sale issued on time/temperature control for safety food due to temperature abuse. In top of ice cream cooler, whipped topping (55F - Cold Holding). Manager stated it has been there since last night. See stop sale.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. In bottom of flip top reach in cooler on cook line, two pans of raw bacon stored over multiple containers of pasteurized liquid egg. Manager moved raw bacon to bottom shelf of cooler.
  • [36-24-5] Hole in or other damage to wall. Hole in wall at drive thru window. Also, wall behind prep table bulging out.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In top of ice cream cooler, whipped topping (55F - Cold Holding). Manager stated it has been there since last night. See stop sale.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets soiled throughout establishment. Also, fan cover in walk-in cooler soiled with dust build up. Also, interior of drawer on cook line used to stored cooked bacon. Also, underside of slushee machine near drive thru window soiled with mold-like substance. Also, around nozzles on drink machine in server area soiled with mold-like substance.
  • [32-12-6] Covered waste receptacle not provided in womens bathroom. Trash can in womens restroom has no lid.
  • [36-73-4] Floor, wall and/or ceiling soiled/has accumulation of debris. Throughout establishment ceiling tiles/vent soiled with dust and black mold-like substance. Also, floor in walk-in cooler soiled with food debris. Also, floor under three compartment sink soiled with debris. Also, floor under all cook line equipment soiled with grease and food debris. Also, wall behind fryers on cook line soiled with grease.
  • [14-69-4] Ice buildup in reach-in freezer and/or walk-in freezer. Ice build up on floor and pipe behind fan unit in walk-in freezer. Bucket may be used on floor to caught water.
  • [35A-03-4] Dead roaches on premises. Observed one (1) dead roach on floor on cook line behind reach in freezer. Manager cleaned up and discarded.
