Observed in reach in cooler below cook line raw beef (51F - Cold Holding) as per operator less than 4 hours. Operator ice over raw beef for rapid cooling.
[06-09-1] Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed comercially processed raw mahi thawing in reduced oxygen packaged during time of inspection. Mahi was still frozen during time of inspection and was removed from packaging.
[08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Observed in reach in freezer across from dish machine raw breaded chicken stored over tostones. Operator stored chicken below tostones.

Observed the dish machine, three compartment sink and hand wash sink removed from current place on plans and placed elsewhere.
