  • [02D-01-5] Working containers of food removed from original container not identified by common name. At dry storage area, bulk containers of seasoning blend and sugar not labeled.
  • [08B-38-4] Food stored on floor. At sandwich make station, open case of buns on floor. Operator moved to dry storage shelf.
  • [11-26-1] No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. 2 employee present with no employee food handler certificates, no proof of employee responsibilities being explained to them. Emailed operator dbpr form in English and Spanish.
