[41-27-4] Wiping cloth sanitizer solution exceeds the maximum concentration allowed. At bar, Sanitizer Bucket (Quaternary 500+ppm). Operator added more water and rechecked at 200ppm.

Steak was sitting above the cooling line of the cooler, operator placed it below the cool line.
