Disposition: Call Back - Extension given, pending
[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Reach in deli cooler cutting boards stained yellow.
[14-09-4] Cutting board has cut marks and is no longer cleanable. Cutting boards on reach in deli coolers.
[51-06-5] A black and orange accordion style sewer hose extension, with a quick disconnect, was observed secured with zip ties to the side rail and attached to a white PVC plumbing line that leads underground. The PVC drain line is located underneath the storage container and is being held up by metal straps that loop around a steel frame and the PVC pipe. The metal straps use bolts to secure it, and hang from underneath the storage containers. Propane gas line bolted to the side of the wooden deck and grey electrical lines bolted to the steel frame that is holding the air conditioning unit and bolted throughout the exterior of the the storage container.
[51-16-7] During the callback inspection, the potable water or wastewater tanks were observed and have not been installed. Outdoor air conditioning unit stored on top of a steel frame thats bolted into the ground, connected to hard piped metal HVAC ductwork along the top of the storage container to provide air conditioning.

[22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Reach in deli cooler cutting boards stained yellow.
[03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Food items stored in pull out drawers underneath flat top grill: raw grouper (55F - Cold Holding); raw grouper (46F - Cold Holding); raw mahi (45F - Cold Holding); raw chicken (46F - Cold Holding) Bags of ice were placed on food items and containers were stored inside the reach in freezer to bring down the temperature rapidly. Re-temped at 3:10pm: raw grouper (52F - Cold Holding); raw grouper (40F - Cold Holding); raw mahi (39F - Cold Holding); raw chicken (39 F - Cold Holding) Discussed monitoring cold holding units to ensure time/temperature controlled for safety foods are properly cold holding at 41 degrees or below.
[12B-07-4] Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Drink stored on top of prep table next to yellow cutting board, stainless steel bowls, and black baskets with parchment paper. Drink moved to bottom of prep table.
[51-16-7] No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. The direct link to the Plan Review page is https://www2.myfloridalicense.com/hotels-restaurants/licensing/plan-review/. No potable water or wastewater tanks were installed and lacks a quick disconnect mechanism for power utilities and sewage lines permanently installed. The MFDV is currently not following plan review provison (both specific and general).
[31A-11-4] Handwash sink used for purposes other than handwashing. Stainless steel pan stored inside the handsink basin. Item was moved.
[51-06-5] Mobile food dispensing vehicle connected to utilities on more than a temporary basis. Permanent utility connections were observed on the MFDV. Observed sewer lines installed directly into the ground beneath the MFDV, HVAC metal tube running from the roof to an air conditioning chilling unit on outside of the truck, and permanent power lines connected to an exterior electrical panel box that serves as the trucks sole source of electricity. PLAN REVIEW SPECIFIC PROVISOS observed on the approved application on MAY 14, 2025: Establishment may not permanantly connect to water and sewer utilities, must install quick- disconnect fittings, and shall not remain connected to the water and sewer utilities for periods longer than 24 hours. Unit must have an adequate method to switch between utility water supply and portable tank water supply.
