[41-17-4] Spray bottle containing toxic substance not labeled. On three compartment sink, clear spray bottle of red liquid not labeled. Operator labeled bottle.

[25-06-4] Single-service articles not stored inverted or protected from contamination. On top shelf of both reach in coolers in kitchen next to server window, to go boxes not inverted. Manager flipped stacks of to go boxes over.
[29-49-6] Standing water in bottom of reach-in-cooler. In middle reach in cooler near server window has standing water at bottom.
[31A-09-4] Handwash sink not accessible for employee use at all times. Upon arrival handwash sink by dish area blocked by pile of dirty pans on floor. Manager moved all dishes from in front of handwash sink.

Manager at time of inspection was unable to provide proof of any food handler training. Manager stated a lot of employees are new hires but a few employees have been here longer then 60 days.
[08B-39-4] Raw fruits/vegetables not washed prior to preparation. In middle reach in cooler near server window, two pans of sliced avocado with stickers still on skin.
[24-05-4] Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. On top of ice machine, one ice bucket not inverted. Manager flipped ice bucket over.
[35A-05-4] Roach activity present as evidenced by live roaches found. Observed 1 live roach in mop sink.
[10-07-4] In-use utensil stored in standing water less than 135 degrees Fahrenheit. In kitchen next to rice cooker, scooper in standing water temperature at 79F.
[22-20-5] Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior top and deflector plate of ice machine has black spots. Also, exterior lip of ice machine under filters has slime build up.
[36-73-4] Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under fryers have grease and food debris build up. Also, wall behind flat top grills soiled with grease.
[41-17-4] Spray bottle containing toxic substance not labeled. In back dry storage room spray bottle of pink liquid not labeled. Manager labeled bottle with sharpie.
[23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets soiled/torn throughout kitchen. Also, hood filters above flat top grill and fryers are soiled with grease build up. Also, top of reach in cooler under flat top grills has black debris build up. Also, sides of fryers have build up of food debris and grease.
