[16-13-5] Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. At bar; 3 compartment sink set up as wash, sanitize, wash. Had discussion with bar tender who set up wash, rinse, sanitize.
[36-01-4] Floor not cleaned when the least amount of food is exposed. Oil under fryers in kitchen.
[36-17-5] Floor tiles missing and/or in disrepair. Kitchen floor has spots where tile is cracking or missing exposing raw concrete.
[51-11-4] Carbon dioxide/helium tanks not adequately secured. By side door 1 unsecured co2 tank.
[41-17-4] Spray bottle containing toxic substance not labeled. Unlabeled spray bottles in kitchen and at bar.
[35A-03-4] Dead roaches on premises. Observed 2 dead roaches. 1 on left side of soda boxes in storage and 1 in front of 2 door reach in cooler in kitchen. Person in charge swept up and disposed of dead roaches.
