Shredded lettuce 59°F at 1:36 PM cooled to 43°F at 2:13 PM, Slice American cheese 61°F at 1:38 PM cooled to 57°F at 2:01 PM, Sliced provolone cheese 59°F at 1:38 PM cooled to 53°F at 2:01 p.m., Shredded cheddar cheese 62°F at 1:39 PM cooled to 53°F 2:02 PM. All items listed above were placed at front line reach in cooler at 12:33 PM. Manager placed all items to walk in freezer for quick cooling. Operator stated front line reach in cooler was broken as of Monday, 5/4/2026 and continued to place Time/temperature control for safety food inside reach in cooler for a maximum of 4 hours. Operator does not have a Time as a public health control form or a means to mark time for Time/temperature control for safety foods during time of inspection.
