- In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed the scoop handle used for cut strawberries not stored above the food. The operator removed the scoop from the strawberries.
- Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On an employee cell phone stored with cloth wrapped silverware behind the bar. The employee properly stored the phone.

- Nonfood-grade containers used for food storage - direct contact with food. Observed black non-food grade plastic containers used to store and in direct contact with multiple dry foods in dry storage.
- Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Observed an employee bagging crispy won ton strips using bare hand contact. The employee was educated on bare hand contact procedures. The operator discarded the won ton strips.
- Toxic substance/chemical improperly stored. Observed a spray pesticide stored near clean plastic containers in the storage area. The operator discarded the spray.
- Stop Sale issued due to food not being in a wholesome, sound condition. Observed an employee bagging crispy won ton strips using bare hand contact. The employee was educated on bare hand contact procedures. The operator discarded the won ton strips.

- Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed the wiping cloth quaternary solution exceeded 500ppm. The operator discarded the solution.
- No paper towels or mechanical hand drying device provided at handwash sink behind the bar. The operator provided paper towels for the hand sink.

- Observed 5 live cockroaches in the back prep area with 3 by the dish machine and 2 by the coolers.
- Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed their dish machine at 25ppm chlorine. Operator called ecolab.
- Cutting board has cut marks and is no longer cleanable. Observed their make table cutting boards heavily grooved.
- Floor not cleaned when the least amount of food is exposed. Observed the floor under the cook line with food debris buildup.

Operator stated foods are served during dinner service and were not in use during breakfast service.

- In-use tongs stored on equipment door handle between uses. There is a set of tongs on an oven door handle. Operator removed the tongs.

See stop sale. Observed their 4hr font counter pizza not time marked. Operator stated it had been made approximately 2hrs ago. Operator marked it accordingly.
- Employee with no hair restraint while engaging in food preparation. Observed the two cooks not wearing hair restraints. Reviewed proper restraints with them.
- Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drinks on the back prep table. Operator moved them.
- Exit door locked. For reporting purposes only. Observed their cashier fire exit door locked. Operator locked it.

Inside walk in cooler: risotto (53 F - Cold Holding) Per operator risotto was prepared the night before. It is easily determined that food did not cool properly. This is a repeat violation from the previous inspection dated 03-06-2026
- Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed emoloyee load dish machine with soiled dishes while wearing gloves and then proceeded to handle clean and sanitised dishes without a glove change and proper hand washing. Discussed with operator the need for hand washing and glove change
- Establishment not maintaining clam/mussel/oyster tags for 90 days. Mussel and clam tags not marked with last date served. This is a repeat violation from the previous inspection dated 03-06-2026
