- Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee handled raw beef then changed their gloves and handled clean equipment without washing their hands. Employee removed the equipment to be sanitized, washed their hands and put on new gloves.

Observed pasta and vegetable salads Employee stated the above items had been prepare onsite more than 24 hours prior , and then proceeded to date items properly to meet standards of inspection.

- Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed their mop sink splitter missing a vacuum breaker on the end.
- Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed their Ahi Tuna not identified as raw on their menu. Operator added asterisks to their menus leading to the advisory.
- Cutting board has cut marks and is no longer cleanable. Observed their make table cutting boards deeply grooved.
- Open dumpster lid. Observed both dumpster and grease trap lids open. Operator closed them.

- Food stored on floor. Observed cooking oil container stored at the kitchen floor. Operator stored cooking oil on crate of the floor.
- Bowl or other container with no handle used to dispense food. Operator discarded.
- Clean equipment/dishware/utensils stored next to handwash/food preparation sink exposed to splash. Observed mop sink next to 3 compartment sink, exposed to splash.

- Toxic substance/chemical improperly stored. Observed alcohol spray bottle on top of prep table on top of blender is located. PIC removed.
- Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Cooked shredded beef was observed at 127°F in hot holding. Per the operator, the beef had been there for less than two hours. Corrective action was taken immediately to reheat the product.
- Required employee training provided by a third-party employee training program and operator unable to provide original certificate for employees trained. Observed copy of food handler certificate for employee, as per person in charge they keep original at the principal office.
