- Pesticide/insecticide labeled for household use only present in establishment. Observed can of RAID on shelf above hand washing sink next to entry of cook line. Operator removed can.
- Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee on cell phone, place phone back in pocket lift ready to eat pizza with bare hand on pizza peel then grab seasoning and spread over pizza, no hand washing sign. Discussed proper glove use and hand washing with Operator.
- Rodent activity present as evidenced by rodent droppings found. Observed approximately 3-5 rodent droppings on expo pass through window, 5-7 under dishwasher, 3-5 under canned goods shelf in dry storage area and 3-5 under empty shelf in center of prep kitchen.
- Dead roaches on premises. Observed 2 dead roaches end of cook line on floor next to reach in cooler. Observed 1 dead roach by back door of kitchen.

- Container of medicine improperly stored. Observed a container of bone vitamins stored on a prep table. Operator removed them,
- No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Owner is in the process of getting more certified managers.

- Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm), operator changed the container of sanitizer, chlorine sanitizer 50ppm.
- Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In the walk in cooler raw ground beef is stored on top of raw sausage. Operator moved items to proper storage order.
- Handwash sink not accessible for employee use at all times. There is a garbage can in front of the hand washing sink by the dish washing are.
- Standing water in bottom of reach-in-cooler. There is standing water in the bottom of the drawer cooler in the pantry area.

- Ceiling tile missing. Observed ceiling tile missing over hostess and cashier stand by front entrance. Ceiling tile missing in dining room over one of the booths. Observed ceiling missing in dining room over a table next to side exit door.

- Raw animal food stored over/not properly separated from ready-to-eat food. Observed employee store raw shell eggs over hollandaise sauce in reach in cooler. Educated employee and they relocated the eggs.
- Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed eggs stored at cook line under no refrigeration. The operator states they have been out less than 30 minutes and relocated to reach in cooler.
- Employee washed hands with no soap. Observed employee wash hands with no soap, stopped and educated employee who properly washed hands.
- Nonfood-grade containers used for food storage - direct contact with food. Observed plastic container for flour mixture stored in a non-food grade container.
