At cook line area in flip top reach-in cooler, cut lettuce (49f, 49F - Cold Holding); shredded cheese (50F - Cold Holding). Items stored in cooler overnight per owner. Ambient temperature 49f. Gaskets torn and standing water observed in bottom section of cooler.
- Raw animal food stored over/not properly separated from ready-to-eat food. At kitchen area in stand up reach-in cooler, raw fish and raw shrimp stored over multiple squeeze bottles with sauces/condiments. Operator removed sauces from reach-in cooler.
- Live, small flying insects found. At main bar, observed 3 flying insects.
- Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. On splitter at mop sink, backflow preventer missing on both ends of splitter. Hose is not hooked to chemical dispenser at time of inspection.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Raw chicken in stand up reach in refrigerator at 47F. Per operator, door has been continually opened and closed for the past 45 minutes. Chicken moved to walk-in freezer to quickly chill and approximately 15 minutes later, raw chicken at 41F. Also, raw scallops at 44F and raw beef at 44F in the walk in cooler. Per operator, door has been open and closed frequently for the last 45 minutes. Door kept shut and approximately 20 minutes later, raw scallops at 41F and raw beef at 41F.
- No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at sushi handwash sink. Operator replenished paper towels.
- No proof provided that some food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Per operator, missing five proof of employee reporting agreements.

Operator stated all items were placed in reach in cooler 3 hours prior. Ambient air thermometer reading 48F, actual ambient air temperature 51F. Operator put ice on all Time/Temperature control for safety foods. Also, in flip top reach in cooler at end of cook line, cut cabbage (50F - Cold Holding); shredded cheese (51F - Cold Holding). Both items placed on ice in top of cooler 3 hours prior but ice not surrounding pan. Operator added more ice around items.
