At the steam table in the kitchen observed cooked refried beans (120F - Hot Holding). Employee stated food was prepared 30 min prior to inspection. Employee stated their intention was to hold food hot as back up container. Discussed proper hot holding temperature, operator decided to cool beans and place in walk in.
- Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. In the menu observed dish, ceviche and ceviche mixto. Operator stated no heat is applied to fish and is marinated in lime juice. Operator began to mark menus.

- Employee washed hands with no soap. An employee washed their hands without using soap. Educated the employee on proper hand washing procedures.

- Spray bottle containing toxic substance not labeled. There is an unlabeled chemical spray bottle on the floor next to the convection oven on the cook line.
- Food-contact surface soiled with food debris, mold-like substance or slime. There is a buildup of food debris on the slicer, around the blade guard and the sharpener.

- Toxic substance/chemical improperly stored. Observed baby wipes stored over the cook line and observed dawn dish soap stored over a container of sugar and mints in the dry storage area. Operator moved the chemicals.
- Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw oysters stored over sauces in a make table cooler and raw shrimp stored over cooked corn in the walk in cooler. Operator moved the oysters and corn.
- Clam/mussel/oyster tags not marked with last date served. Observed none of their tags marked.
- Accumulation of debris on exterior of warewashing machine. Observed a buildup of debris on top of the dish machine.
